Cascade Neuropsychology only accepts Medicare and Blue Cross Blue Shield.

Out-of-Network Services

Although we may be out-of-network with your insurance plan, the reimbursement process is typically a single form submission with your receipt of payment. Most health insurance plans reimburse you within 45 days. Please fill out the intake form to get an estimate of your reimbursement.

We are unable to accept Medicaid patients at this time.

Prior Authorization

Some insurance plans require prior authorization for neuropsychological evaluations. Call your insurance company and ask if you need prior authorization.

Ask your insurance representative to fax it to 360-785-2016 or simply give us the authorization number when it’s convenient for you.
